Host Home Program:
The Host Home program gives people with intellectual disabilities a safe and supportive place to live. They live with a caring person or family who does not have a disability. Over time, they become a valued part of the household. This helps them avoid the stress of having different staff come and go.
Host Home Providers:
Host Home Providers with Progressive Community Network (PCN) are Independent Contractors. As a provider, your job is to offer support, guidance, and care based on what each person needs. This can include helping with life skills like making a budget, cooking, doing laundry, staying clean, and managing emotions. Some people may also need extra help with things like moving around, bathing, or using the bathroom. You’ll work with a team of people who care about the person’s well-being—and you and the person you support are both important parts of that team.


Host Home Activities:
As a Host Home Provider, you’ll need to keep daily notes about the care and support you give. Basic services include:
- Providing a safe and comfortable home
- Helping with medications
- Giving daily support based on the person’s needs
- Taking part in team meetings to talk about the person’s care
Community Requirements to Be a Host Home Provider:
- Be 21 or older, responsible, caring, and patient
- Use good judgment, speak clearly, and stay organized
- Have a home or apartment (owned or rented) with a private bedroom for the person you’ll support and access to shared spaces
- Complete all required training from the state, PCN, and for the individual’s needs
- Pass background checks, including checks for criminal history, sex offender registry, OIG, and child or dependent abuse registry
