Host Home Program
The Host Home program provides individuals with developmental disabilities a supportive, stable home environment. This model allows a person to live with a family or individual without disabilities, becoming a valued member of the household and moving away from frequently changing support staff.
Host Home Providers with Progressive Community Network (PCN) are independent contractors. As a Host Home Provider, your role includes offering supervision, guidance, and training tailored to the individual’s unique needs. Training may cover various life skills, including budgeting, cooking, laundry, personal hygiene, and behavioral coping strategies. Some individuals may also require more intensive support, such as assistance with mobility, bathing, or toileting. These needs are addressed collaboratively with an interdisciplinary team dedicated to the individual’s well-being, of which you and the person served are key members.
As a Host Home Provider, daily documentation of all training and activities is required. The minimum services to be provided include:
- Room and board
- Relief staff
- Daily support services
- Transportation
- Participation in ongoing training and meetings
Requirements to Become a Host Home Provider
To qualify as a Host Home Provider, you must meet the following requirements:
- Be at least 21 years old, responsible, compassionate, and patient
- Demonstrate good judgment, clear communication, and organizational skills
- Have a home or apartment (owned or rented) with a private bedroom for the individual supported and access to shared living spaces
- Successfully complete a background check (including criminal history, driving record, sex offender, OIG, and child-dependent abuse registry checks)
- Complete state-required training (e.g., CPR, dependent adult abuse reporting) and any additional PCN training
