What primary services do you offer?
We offer in-home SCL residential services to adults 18 and over on HAB, ID, and BI waivers. PCN also provides Host Home services to individuals seeking a different living arrangement than the traditional HCBS SCL setting.
What is 24/7 SCL?
In-home residential services provide a variety of supports for a person, which include supporting a person to live in their own home or apartment. Staff assist persons supported with learning new skills, household chores, personal hygiene, meal preparation, shopping, social and adaptive skills, and more. Residential supports are 24/7 and are designed to promote and increase independence within one’s own home. Our primary goal is to support people in maximizing their abilities and increasing participation in life.
What is a Host Home?
Host Homes are a living arrangement in which a person supported lives with an Independent Contractor and receives services while cohabitating with a host individual or family.
What do you specialize in?
We specialize in providing the highest quality and supportive care to individuals with disabilities and chronic mental illness.
What are the requirements to become a Host Home Provider?
- Valid Driver’s License
- Auto Insurance
- Desire to provide in-home 24/7 support to individuals with disabilities
- Completion of required training
- Completion and passing of background check for independent contractor
- Completion and passing of background checks for all individuals in the home 18 and over
- In-home inspection by PCN
Who can receive services?
Our services can be provided to anyone on HAB, ID, and BI waivers seeking SCL services.
