Requirements to Become a Host Home Provider

To qualify as a Host Home Provider, you must meet the following requirements:

  • Be at least 21 years old, responsible, compassionate, and patient
  • Demonstrate good judgment, clear communication, and organizational skills
  • Have a home or apartment (owned or rented) with a private bedroom for the individual supported and access to shared living spaces
  • Successfully complete a background check (including criminal history, driving record, sex offender, OIG, and child-dependent abuse registry checks)
  • Complete state-required training (e.g., CPR, dependent adult abuse reporting) and any additional PCN training
