Host Homes: A Place to Belong
Host homes give people with disabilities a chance to live in a caring and family-like home. Instead of living in a group setting, they become part of a real home where they can feel safe and included.
People who live in host homes get help that fits their needs and goals. They can join in everyday things like meals, chores, and fun activities in the community. They also get to build friendships and feel like they belong.
Host homes do more than just provide supportβthey help break down stereotypes and build caring communities where everyone matters.

What primary services do you offer?
PCN gives full-time support to adults 18 and older on the HAB, ID, and BI waivers through our Host Home program. This service provides a safe, family-like home where people get help with daily life, joining in the community, and becoming more independent. Support is based on each personβs needs, goals, and strengths.
What do you specialize in?
We give caring, personalized support to people with disabilities and mental health needsβfocusing on respect, independence, and building strong connections.
What is 24/7 SCL?
Host Home services give people a safe, family-like place to live with support available 24/7. They live in the private home of a caring provider and get help with daily skills, personal care, chores, and being part of the community. The goal is to help each person become more independent and enjoy a better quality of life.
What You Need to Become a Host Home Provider:
- AΒ valid driverβs license
- Auto insurance
- AΒ willingness to provide 24/7 supportΒ to someone with a disability
- Complete required training
- Pass a background checkΒ (for you as an independent contractor)
- All adults in your home (18 and older) must also pass background checks
- AΒ home inspection by PCN
What is a Host Home?
Host Homes are a type of living arrangement where a person lives with an Independent Contractorβsomeone who is trained to give supportβand shares a home with them or their family.
Who can receive services?
Our services are for people on the HAB, ID, or BI waivers who need Supported Community Living (SCL). SCL helps people learn daily living skills, be more independent, and take part in their community.
