Host Homes: Creating Inclusive, Family-Centered Care
Host homes offer individuals with developmental disabilities or special needs the chance to live in a nurturing, family-like environment. This model surpasses traditional care. It places individuals in the heart of a home. They form meaningful relationships and fully engage in daily life.
By living with a host family, individuals get personalized support that honors their preferences, strengths, and goals. They contribute to household routines, join in community activities, and build lasting connections.
Host homes don’t just support individuals. They foster inclusion and challenge stereotypes. They build stronger, more compassionate communities where everyone has the chance to thrive.

Our mission, guided by the motto of “Care and Compassion,” is to deliver outstanding care to individuals of all abilities, and to foster complete inclusion across all facets of life.

Progressive Community Network (PCN) began with the desire to offer services with a different mindset. Built from the ground up, PCN started in December of 2021 serving two individuals on the ID waiver. Today PCN serves 40+ individuals between the ID, BI, and HAB waivers and continues to expand.
Proud Member of IACP
Progressive Community Network has been a proud member of the IACP since 2023.
IACP supports Iowa community providers so they can fully support Iowans in need of behavioral health and disability services. More than 130 providers across the state look to IACP as a trusted resource in their work to help those they serve to lead more productive and fulfilling lives. This is reflected in IACP’s organization’s mission and vision.
